Community Gateway has been providing support to people with disabilities or who are aged to remain living independently at home since 1989.
If you are aged over 65 (or over 50 years if you are identify as Aboriginal or Torres Strait Islander) Community Gateway can assist you with any level of support you may require to maintain your independence.
We understand that the changes in aged care are challenging. We can help you to access My Aged Care online. Our experience, strong relationships and networks in the local aged care sector help us support you to make informed decisions and choose the supports you need to live the life you want. Should you require assistance to access residential respite or permanent aged care, we will be there to support you through this transition too.
Community Gateway also has innovative programs to help meet your desire for community participation, health, wellbeing and fun! You may like to access social bus trips, computer learning labs, our men’s group or ladies’ day out.
We focus on understanding your circumstances and preferences and ensure you direct your own care. With us you will have more say in the support and services you access, how they are delivered and who delivers them to you.
We will talk with you about your needs and goals and develop your individual support plan with you, and you decide how much involvement you have in managing your own care.
We can assist you to access entry level support under the Commonwealth Home Support Program through the Australian Government’s My Aged Care and can provide you with services including domestic assistance, personal care, social support, help with your shopping or help to access medical appointments.
If you have been assessed as requiring a higher level of support and some help with support coordination, we are able to provide that support to you too under the Australian Government’s Home Care Package program. If you need assistance to access this program we can help to arrange an assessment by the Aged Care Assessment Team. Whether you have been assessed as requiring a Level 1 package (basic level of care needs), a Level 2 package (low level), a Level 3 package (intermediate level) or Level 4 package (high support needs) we will provide you with case management, support coordination and services to stay at home for as long as possible.
Under a Home Care Package you may also choose to access other supports including aids and equipment, allied health assessments and services, home modifications and nursing care. We have experience working with people with dementia or other special needs and can also provide support to younger people with dementia. We let you know how your funding is spent and provide you with regular statements. We will review your package with you regularly to ensure your services continue to meet your needs.
Community Gateway assists people to live at home and remain active in their communities through our Wellbeing Services. The Wellbeing Service team provides flexible, high quality and cost effective services 24 hours a day, 7 days a week and operate in the Illawarra, Shoalhaven and Macleay Valley.
